Columbia County Community Support Program

The Columbia County Community Support Program (CSP) provides an array of community-based services to adults living in Columbia County who are affected by serious and persistent mental illnesses (such as schizophrenia, depression, bipolar disorder, delusional disorder and schizoaffective disorder) and who may be at risk for institutionalization, frequent hospitalization or dysfunction in quality of daily living. Despite their psychiatric difficulties our clients and consumers find that with appropriate support they are able to manage their illness effectively and lead happy, rewarding and creative lives. Our mission is to help our consumers be as independent and productive as possible.

We believe that people who have mental illnesses should be treated with dignity and respect, that they have the same needs, aspirations, rights and responsibilities as other citizens, that they should have access to the opportunities available to others, and that essential services should be readily available to them.

 

Clinical Services

Our services are provided by a skilled and highly trained staff certified by the Division of Community Services of the Department of Health and Human Services in compliance with State of Wisconsin statutes.

The CSP is unique in that we provide services our in the community, where residents live and work.

The services offered through the CSP are many, and include 24-hour crisis intervention, symptom management, supportive counseling, medication management, psychiatric and psychological services, individual, group and family psychotherapy, rehabilitation services, employment and vocational services, activities of daily living training, social and recreational skills training, support services (access to services to meet physical or dental needs, legal needs, necessary transportation, financial support, and living accommodations). These services, for most clients in the program, tend to be long-term.

Every individual receiving services through the CSP benefits from an individually tailored treatment plan that is developed to meet each consumer’s specific needs. This individualized treatment plan allows everyone involved with the client to evaluate the impact and efficiency of services that are provided and progress toward treatment goals, enhances communication between all direct service staff, and ensures that services are well defined and consistently implemented. The individualized treatment plan is developed within 30 days following admission or transfer into the program and is valid for six months. Treatment planning occurs directly with the client. All treatment objectives are written to support treatment goals and are written by the case manager and the client. The case manager reviews the treatment plan on a monthly basis with the client. Treatment is reviewed at least quarterly with the entire treatment team during the interdisciplinary team meeting. The following participants are involved in the development of the treatment plan: the client, the client's guardian, family members and significant others, the treating psychiatrist, the CSP case manager, other CSP program staff that may be involved in the delivery of services to the client and other community based service providers.

A program psychiatrist supervises all services provided by the CSP.
